MicroSort® -
Using a Local Collaborating Physician
XMicroSort® is an FDA approved clinical trial. MicroSort® has accepted numerous physicians to collaborate with us in this trial. If you do not see your physician on our collaborator list, please feel free to call us at 703-876-3897 to locate your physician or a physician local to your area. Not all collaborating physicians are listed on the collaborator web page.
The following steps should be taken:
Step 1: Your IVF center physician will need to enroll directly with MicroSort®, prior to your procedure. Enrollment may take several weeks. The physician may call 703-876-3897 and speak to a MicroSort® collaborating services representative. Once your physician is registered go to step 2.
Step 2: Download the medical history form and the patient registration form to your computer and print it out. (If you do not have Acrobat Reader on your computer, you may download a free copy now. After downloading the file, return to this page and click on the documents listed above) Fill out the forms and fax it to 703-698-7293. If you need copies of these forms sent to you, please call 1-800-277-6607. Once your forms are received, a MicroSort® customer service representative will review your forms and call you to schedule a phone consultation.
Step 3: During your phone consultation a MicroSort® clinician will talk to you about your family history and reproductive issues. If the preliminary information allows entry into the MicroSort® clinical trial, you will be sent a packet of forms for your signature and return. Along with the signed forms, you will also need to send or fax a copy of infectious disease testing results for HIV 1, HIV 2, HepBsAg and Hep C antibody for you and your spouse. These results must be dated within the last year from the sort date. A semen analysis is recommended and may be required, depending on your particular situation.
Step 4: Once all of the paperwork is received and approved (for completeness and accuracy) a MicroSort® clinician will contact you to schedule a date and time to arrive at the Fairfax,Virginia MicroSort® Laboratory to leave a semen specimen for sorting.
Step 5: After the specimen is sorted, the MicroSort® Laboratory will arrange for shipping to your local collaborating physician for your IVF procedure.
Step 6: After the procedure, your physician must send in a case report form to advise MicroSort® of the treatment cycle and the outcome. In the event that pregnancy is achieved, MicroSort® or your collaborating physician, will follow up with you during the pregnancy, request birth records after the birth and pediatric records when the child turns 1 year of age. |
